Benefit & Hazelwood Forms

Announcement: The ACC Veteran Affairs office is excited to announce that we have streamlined our VA document collection process by implementing StudentForms.

To start the process of utilizing your VA benefit, contact the ACC Veteran Affairs office at [email protected] or call 512-223-4636 and press 5 for FA/VA and 2 for VA to activate your StudentForms for VA documents.

Steps for submitting VA Benefit and Hazelwood Applications to ACC.

  • Request a Student Forms account activation through [email protected] or phone (512-223-4636 option 5 for FA/VA and then 2 for VA).
  • Complete and sign your VA in-processing checklist (required each semester) located within StudentForms
  • Upload any needed documents to activate your benefit. Links for locating these documents are found below. Hazelwood applications will auto-populate in your account once you indicate on your checklist you intend to use this benefit.

Complete form when attending ACC and taking a class at a different institution. ACC will be the school where you take a majority of your classes.

The Hazelwood application can be located within StudentForms, once you indicate you will be using this benefit on your completed in-processing checklist.
